Join Navigate Home Health Care and help deliver personalized, compassionate care directly in patients’ homes. We are seeking a dedicated PRN Registered Nurse to serve patients throughout Sarasota County and help them maintain comfort, independence, and quality of life.

About the Role

As a Home Health Registered Nurse, you will provide skilled nursing care in accordance with each patient’s plan of care. Your work will include comprehensive health and psychosocial assessments, monitoring patient conditions, coordinating services, educating patients and families, and providing direct nursing care.

Key Responsibilities

✓Coordinate total patient care through comprehensive health and psychosocial assessments, ongoing monitoring, preventive care, education, and service coordination.
✓Evaluate the effectiveness of nursing services provided to patients and their families.
✓Complete admission, transfer, recertification, resumption of care, and discharge OASIS assessments.
✓Coordinate with physicians regarding changes to patient care plans and implement approved updates.
✓Coordinate services with Home Health Aides, Physical Therapists, Occupational Therapists, Speech Therapists, Medical Social Workers, and other members of the care team.
✓Participate in case conferences, staff development meetings, care planning, and discharge planning.
✓Submit clinical documentation, progress notes, and other required records accurately and on time.
✓Maintain current knowledge of drug therapy and professional nursing practices.
✓Maintain compliance with federal, state, Medicare, Medicaid, and accreditation requirements.
✓Participate in ongoing professional development and continuing education.

Qualifications

✓Graduate of an accredited School of Nursing.
✓Current Registered Nurse license in the State of Florida.
✓One or more years of experience in a community health agency, home health agency, or hospital setting preferred.
✓Knowledge of Medicare and Medicaid guidelines.
✓Working knowledge of home healthcare, professional nursing practices, and required clinical documentation.
✓Strong organizational, communication, and time management skills.
✓Current CPR certification. Online certification is not accepted.
✓Successful completion of a criminal background check.
✓Valid driver’s license and ability to travel to patients’ residences.

Physical Requirements

This position requires travel to patients’ homes and the ability to access residences that may not be routinely wheelchair accessible. Hearing, eyesight, and physical dexterity must be sufficient to conduct patient assessments and provide care. Physical activity may include walking, sitting, standing, stooping, turning patients, and lifting patients when required.
