We are seeking a dedicated full time Physical Therapist to serve patients throughout Lee County, including Bonita Springs and Estero, and help them maintain mobility, independence, and quality of life.

About the Role

As a Home Health Physical Therapist, you will provide high quality physical therapy services to patients in the comfort of their own homes in accordance with physician orders. You will assess patient needs, develop and implement individualized treatment plans, coordinate with physicians and other members of the care team, and help patients improve their mobility and functional independence.

Key Responsibilities

Provide initial and ongoing comprehensive assessments of patient needs, including Outcome and Assessment Information Set assessments.
Record and report patient responses to treatment and any changes in condition to the physician.
Communicate with physicians at least every 30 days, or whenever changes occur, regarding patient status and obtain additional orders when necessary.
Communicate with other members of the care team, including Home Health Aides, and provide appropriate instruction related to the physical therapy plan.
Assist in the development and implementation of interdisciplinary patient care plans as they relate to Physical Therapy.
Assist physicians in evaluating patients using diagnostic and prognostic muscle, nerve, joint, and functional ability testing.
Educate staff regarding established physical therapy plans and appropriate patient care procedures.
Teach, supervise, and counsel patients and their families regarding physical therapy programs.
Instruct patients in the proper care and use of wheelchairs, braces, crutches, canes, prosthetic devices, and orthotic devices.
Arrange and provide equipment required to carry out the patient’s plan of care.
Arrange outpatient services for procedures that cannot be provided in the patient’s home.
Prepare assessments of patient conditions along with periodic evaluations and summaries.
Supervise Physical Therapy Assistants and Home Health Aides in accordance with Agency policy.
Complete clinical and progress notes on the date of each visit and incorporate documentation into the patient’s record within 48 hours.
Participate in case conferences, physical therapy staff meetings, Agency activities, and in service programs as required.
Supervise Home Health Aides every two weeks when physical therapy is the only professional discipline involved and supervise Physical Therapy Assistants at least monthly.
Participate in discharge planning with staff, patients, and physicians and complete Physical Therapy Discharge Summaries.

Qualifications

Successful completion of a Physical Therapist education program approved by the Commission on Accreditation in Physical Therapy Education, a successor organization, or an approved equivalent international program.
International education programs must be determined substantially equivalent to United States entry level Physical Therapist education by an approved credential evaluation organization.
Successful completion of a Physical Therapist examination approved by the State of Florida.
Current Physical Therapist license or registration in the State of Florida.
At least one year of institutional or home health experience.
Successful completion of a criminal background check.
Current CPR certification. Online certification is not accepted.
Valid driver’s license and ability to travel throughout Lee County.
Ability to communicate effectively in English.

Physical Requirements

This position requires driving and the ability to access patients’ homes, including residences that may not be routinely wheelchair accessible. Hearing, eyesight, and physical dexterity must be sufficient to perform patient assessments and provide care. The role may occasionally require bending, stooping, reaching, moving patient weight of up to 250 pounds, and lifting or carrying up to 30 pounds.
